Role Overview:

The Physician Assistant (PA) will deliver medical care to homeless patients as part of a multidisciplinary Integrated Mobile Health Team (IMHT). Based primarily at the Rose Clinic, this role involves providing primary care services to low-income patients, including those with chronic conditions, pediatrics, gynecology, and the homeless population. The PA will work independently in assessment, management, and follow-up of patients, and participate in case conferences and street outreach.

Key Responsibilities:

• Provide primary care services to low-income, chronically ill, pediatric, gynecology, and homeless patients.
• Conduct patient assessments and manage follow-up care independently.
• Participate in case conferences and street outreach as part of the IMHT.
• Supervise clinical support staff and collaborate with multidisciplinary teams.
• Create and maintain patient treatment plans and documentation.
• Operate a computer to obtain lab results, log clinic visits, and input/retrieve data.
• Communicate effectively with patients, their families, and the healthcare team.
• Determine when to function independently or seek physician intervention.

Qualifications:

• Required:
  • Graduated from an accredited PA program.
  • Master’s degree in Physician Assistant Program (preferred).
  • CA license to practice as a Physician Assistant.
  • National Certification as PA.
  • BLS certification from American Red Cross or American Heart Association.
  • Proficiency in computer operations for lab results, clinic logs, and data management.
  • Knowledge of disease processes and treatments for chronic diseases, pediatrics, and gynecologic issues.
  • Demonstrated ability to supervise clinical support staff.
  • Strong interpersonal and communication skills.
  • Ability to perform patient assessments and take appropriate actions.
  • Organizational skills to create and manage patient treatment plans.
  • Ability to work independently and set priorities.
  • Skill in writing patient notes, progress reports, and other documentation.
  • Ability to determine when to seek physician intervention.
• Preferred:
  • Skill in communicating in medical Spanish.
