Job Summary
We are excited to offer an opportunity for a Senior Community Nurse or District Nurse to join our Integrated Teams in Redbridge. This role requires either a District Nurse certificate or two years of experience as a community nurse. The successful candidate will help lead a team that provides high-quality nursing care to patients and their families in the community, all while supporting the values of the Trust.
You will be part of a team responsible for providing care to a caseload of patients within a geographically aligned area to GP practices. This role will involve delivering individualized care, collaborating with multidisciplinary teams, and working autonomously. The role also offers leadership opportunities as you will be guiding a community nursing team. There are opportunities for development for nurses currently undertaking the Band 6 Development Program.
Main Duties
Lead and manage a community nursing team aligned to specific GP practices.
Oversee the provision of nursing care to patients, managing a caseload and ensuring high standards.
Work autonomously and as part of the integrated primary/community health care team, collaborating with GPs and other professionals.
Supervise and mentor pre-registration students.
Provide leadership and guidance to community nursing teams, helping them achieve the Trust’s goals and values.

The role comes with a High-Cost Area Supplement of 15% of the basic salary, with a minimum payment of £4,551 and a maximum payment of £5,735 (pro-rata for part-time staff).

Qualifications
Essential:
Current NMC Registration as a Registered Nurse (RN)
Evidence of professional updating
Agreement to undertake required modules in the Band 5/6 Development programme
Ability to study at degree level
Desirable:
Relevant post-registration course or experience (e.g., Palliative Care, Diabetes, Tissue Viability)
Mentor qualification
Experience
Essential:
Experience in team working and managing an effective team
Experience of interagency and collaborative working
Experience in teaching and assessing individuals
At least 2 years of community experience
Desirable:
3+ years post-registration experience
Experience in managing staff conflict and challenging situations
Evidence of ensuring practice is based on sound evidence and clinical audit
Knowledge
Essential:
Awareness of NHS Plan, NSF, and clinical governance priorities
Familiarity with the NMC Code of Professional Conduct
Good clinical skills, including assessment
Understanding of chronic disease management and rehabilitation in the community
Skills
Essential:
Basic IT skills
Excellent interpersonal, communication, and leadership skills
Strong organization skills and ability to work independently
Ability to prioritize and manage tasks effectively
