Job Summary
NELFT is looking for a Band 6 Community Dietitian to join their team, providing expert nutritional support to patients across a variety of community settings. This full-time role (37.5 hours per week) offers an exciting opportunity to make a difference by managing chronic conditions, promoting healthy eating, and supporting overall wellbeing in patients’ homes, care homes, community hospitals, and more.
As a Community Dietitian, you will work closely with multidisciplinary teams and patients, delivering both individual and group nutrition interventions. You’ll also support the development of the community rehabilitation team, assist with virtual wards, and contribute to the training of other health professionals.
Main Duties
Provide nutritional advice and interventions to adult patients in community settings (home visits, clinics, nursing homes, etc.).
Manage a caseload of patients with diverse needs, including chronic conditions.
Deliver training to other health professionals and support virtual frailty wards.
Organize and prioritize clinical and non-clinical workloads, including follow-up appointments and home visits.
Liaise with community rehab teams and nurses to ensure seamless care delivery.
Contribute to the development of services in collaboration with other teams.
What We’re Looking For
Essential:
Registered Dietitian with HCPC registration.
Experience working in community or clinical settings (Band 5 or above).
Strong communication and interpersonal skills.
Ability to work autonomously and as part of a team.
Passion for promoting public health and nutrition.
Must hold a valid driver’s license and have access to a vehicle.
Desirable:
Experience working in a community dietetic environment post-qualification.
About Us

This role offers a High Cost Area Supplement of 5% of the basic salary, with a minimum of £1,258 and a maximum of £2,122 per year (pro rata for part-time staff).

Person Specification
Essential:
Must hold a valid driver’s license and have access to a vehicle.
Experience working in an NHS setting.
Desirable:
Post-qualification experience in a community dietetic environment.
